          ANNEX D (Service Support) to OPLAN 5-90



          3. Medical Evaluation and Hospitali--ation

              a. 2 CSH hospitals and I Evac hospital to be established in
          Corps sector.

              b. COSCOM Medical Regulatory detachment will tract,
          hospitalized personnel by name until evacuation from theater or
          return to unit.


          4. GREGG


              a. Collection points located at each BSA and DSA. Corps
          establishes 4 collection points in Corps rear. Units use closest
          attention point.

              b. Temporary burial only iuthorized for chemical
          contamination and combat requirements.

              C. Return of personnel effects responsibility of unit
          commanders. Return through APOE at KKMC.

          5. Coordinating Instructions

              a. Submit requisitions to 2nd ODSCOM ACSMAT/MMC rep co-
          located with 731st Maint Bn after arrival at Log Base A. POC:
          LTC McKennon.

              b. Reports submitted per Bde FSOP after closure in TAA.

              C. Bdo S-4/BMO (rear) rep located at        until all Bdo
          equipment arrives at port. POC: CW2 Dicks.
