SUJECT: Outbreak of Diarrheal Disease in Staff Members of
the 410th Evac Hospital
A second outbreak of gastrointestinal disease has been reported
at the 410th Evac Hospital. This outbreak occurred among staff
members of the 410th Evac Hospital. The first cases were reported
Friday night, 5 April and Saturday, 6 April 1991. Symptoms
lasted 3 to 4 days and included abdomenal cramping, diarrhea,
and some nausea and vomiting. Generally no high fevers were
noted in these patients. At the present time approximately
70 to 80 patients have been examined with symptoms, 2 of these
have been admitted to the hospital. 40 stool samples from these
patients have been found to contain Entamoeba histolytica cysts.
Almost all patients were staff members who worked in the hospital.
MAJ Browning, the attending physician, reported that a pattern
had been noted by physicians. The first cases were seen in the
TMC staff, then in the EMT staff, followed by ICW #1 (primary
ward for diarrheal disease patients) and Ward # 2 (overflow
ward for diarrheal cases). Only a few cases have been noted
in non-hospital staff members, 3 from the dining facility and
a couple from the motor pool. Therefore, there is some concern
that the disease may have been transmitted from patients, as
opposed to foodborne in origin.
The hospital laboratory did procure some lettuce that had not
been served by the dining facility. The lettuce was chopped,
and rinsed with sterile water. The rinse water was then examined
and found to contain Entamoeaba histolytica cysts. The dining
facility had originally obtained the lettuce from the 229th
DSU.
Dr. Browning also noted 98% of the cases of diarrheal disease
