             ANNEX E (MEDICAL) TO FORSCOM DEMOBILIZATION PL
             OPERATION DESERT SHIELD                        AN (FDP) FOR

                     (2) All USAR soldiers will be asked if a medical
             examination is desired and medical records annotated.
                     (3) All USAR soldiers requesting a medical examination
             will undergo such examination.
                     (4) Medical records Of soldiers not requesting a
             medical examination, to include records from AOR, if applicable,
             will be reviewed by a physician or physician assistant to
             determine if medical condition/trea@m-ents received while on
             active duty warrant a medical examination.
                     (5) Medical records will be screened to determine if
             soldier has been in medical surveillance program because of
             hazardous job exposure. Appropriate clinical examinations and
             laboratory tests will be conducted prior to separation even if a
             complete medical exam is not r @ ired.
                b. Dental Screening.
                    (1) Dental record will be reviewed to ensure all dental
             care provided during active duty period is properly annotated.
                    (2) If period of active dutv is greater than 180 days,
             refer to AR 635@10, para.2-4e for sp;cific requirements prior to
             separation.

               C. Health Counseling.
                   (1) DMS medical personnel will instruct soldiers on".ithe
             right to a medical examination prior to separation and the
             advAntages/disadvantages of undergoing such an examination.
                   (2) DMS"personnel representatives will Provide soldiers
             with specific guidelines on how to report and receive medical
             treatment for any DESERT SHIELD related illnesses or
             conditions.
                   (3) DMS medical nersonnel will be informed of the
mptoms associated with the following infectious diseases, all
             of which have long incubation periods, and which may not become
             obvious until after demobilization. They are listed in order of
             increasing incubation times.
