        DISCUSSION: Deployed dental teams had no standardized forms or method to
        record dental treatment or productivity. A reporting procedure is required.
        
        RECOMMENDATION: Use AF Form 644B to record dental treatment. Original copy to
        be used as a field dental record, and duplicate to be used to credit procedures
        completed.
        
        ACTIONS REQUIRED: Include use of AF Form 644B in ACC Reg 400-10 for recording
        dental procedures. .
        
        ACTIONS COMPLETED: ACC Reg 400-10 has been revised to include guidance on use
        of AF Form 644B.
        
        STATUS: Closed.
