&ntibatics were discontinued, 3. DOXYcyclins and ciprotloxacin
may have offered some advantage over penicillin in this
(inhalation anthrax) setting, and 4. c=bined antibiotics and
vaccination in the post-axpcsurs period resulted in the bast
s@ival.
A simplified review of pathogenesis coupled with speculation
about mechanism of antibiotic activity might shad light on the
observed findings. Anthrax organisms are most effectively
delivered by inhalation in the form of spares. inhalation of
these spores results in sesding of terminal respiratory
branchioles. spores than are phaqo@osed by alveolar
macrophaqes and transported to lymphatic organs in the chest
(pulmonary and mediastinal lymph nodes) and elsewhere. There,
the spores germinate, becoming "vegetative" organisms which
release toxins and exert adverse physiologic affect$. With
continued replication of these ''vegetative'' bacilli, increasingly
larger numbers of organisms and their toxins circulate. Finally,
the host in overwhelmed and de&= ensues. with administration of
appropriate antibiotics, vegetative forms (= spores) are
killed. in some cases, this may be adequate for a "cure." in
many others, however, additional spores probably remain do=ant
until antibiotics are discontinued. in these individuals,
residual spores then germinate, and the replicative process
begins answ. Theoretically, these persons could remain on
antibiotics for prolonged periods, and survive. in practice
