At 24:00 of 8/7/1986, the enemyÕs artillery bombed the sites of our battery in the sector of Kalala. 
Big fires erupted in the piles of ammunition and shelters. As a result, the below mentioned 
chemical equipment were burnt and destroyed. They were in the custody of Fire Sergeant Ghazy 
Hashem Kazem who is a member of our battery.

Upon conducting the investigation and hearing the testimony of each of:
1. W. O.	Kamel Abdalla Juleib		of battery 3				witness
2. Platoon / 8	Mohamad Salem Saadoun	as sergeant of a platoon		witness
3. Sergeant	Ghazy Hashem Kazem		who is responsible for the equipment

the investigation committee confirmed the following:
a.	At 24:00 of 8 / 7 / 1986, the enemyÕs artillery bombed the sites of Battery 3 of our  
battalion at the sector of Kalala (Kardama battle). Big fires erupted in the piles of ammunition and 
shelters and the below mentioned chemical equipment were burnt and destroyed. They were either 
burnt down to ashes or were left in the battlefield due to the failure of finding them. They were 
under the supervision of Ghazy Hashem Kazem of the above mentioned battery who was busy 
moving the artillery away from the fires and extinguishing fires. Therefore, he was unable to save 
his chemical equipment.
b.	Below is a table of the value of the above items:

c.	The process of burning and destroying the above items was caused by the enemyÕs artillery 
bombing during the battle due to unintentional neglect.

d.	Therefore, the investigation committee ruled that Fire Sergeant no. 98438 Ghazy Hashem  
Kazem was negligent under article 139 of the military law and has to be fined with an amount of 
14,000 dinars under Article 32 of the rules.

e.	The ruling was carried out upon a unanimous agreement and was made on 12 / 26 / 1986.
