2. Disposition of medical/dental records
a- Outpatient field files. in the area of operations
(AO), a folder of outpatient information will consist, in
part, of the HSC Test Form 621-R (individual medical
History) t Si? '600 (chronological Record of Medical Care),
SF 558 (Emergency Care and Treatment Record), SF 603
(Dental), or DD Form 1380 (Field Medical Card). upon
termination of operation Desert storm and return of forces
to -home station/mob station, outpatient field files will
be returned to home station/mob station records custodian
and combined with health record/dental record.
b. Inpatient Treatment Records (ITR). ITRZ on all
patients dispositioned from AO facilities (NOT
TRANSFERRED) during the month will be forwarded to the
operation Desert Storm Records Holding Area at
USAISC-Pentagon (ASQNS-OP-F). ITRs of patients
transferred to US military medical treatment facilities
(MTF) outside the AO will become part of the receiving
M'rF's ITR.
