REFERENCE. JCS PUB 6, Part 13, Volume 11
1. PURPOSE. The Logistic Incident Report is designed to
provide the Commander, Third United States Army (USARCENT) with
a close to real time summary of any logistic incident that
critically impairs logistic support of operations. An incident
is c@onsid@ered critical if logistic effectiveness is impaired to
the extent that it directly and immediately constrains a major
combat operation.
2. SUBMITTED BY. Each Major Subordinate Command as required.
3. SUBMITTED TO. USARCENT Forward AFRD-GDO.
4. WHEN SUBMITTED. Upon occurrence of a critical logistic
incident.
5. METHOD OF TRANSMISSION. Report immediately by secure phone
and Eo-n-firm b@@im@medi@a@E-e-precedence message or, if the
headquarters are collocated, by handcarried message.
Confirmation message should be transmitted NLT six (6) hours
after initial incident report. Imposition of MINIMIZE will not
change method of transmission.
6. CLASSIFICATION. As appropriate, but not normally higher
than secret.

PART I - INCIDENT DESCRIPTION
1. LOCATION (Incident "as oħ" Date/'I'ime).
2. DESCRIPTION OF INCIDENT (Be specific: capacity lost,
etc).
3. ESTIMATED GET-WELL DATE.
PART II NARRATIVE. Provide brief narrative on anticipated
operational impact, corrective actions underway, and USCENTCOM
assistance required if any.
