        3. NOTES.

        Note 1. Location report should be as of the same day and time
        as the LUGSTAT Report. If it is the same, state "same." if
        it's not the same, indicate date and time of data.

        Note 2. A coordinated logistic/personnel staff input.
        indicate in parenthesis significant near term population
        changes.

        Note 3. Outline briefly current situation, status of
        termitial/pipeline operations (if applicable), and planned
        actions for the next 5 days.

        Note 4. identify any existing or potential significant
        sustainment problems. If problem(s) noted, specify anticipated
        mission impact, status of corrective action (to include
        estimated get-well date), and assistance needed, if any.
        Continue to report on status daily until each problem is
        satisfactorily resolved. If there are no problems, state
        .#none."

        Note 5. Calculate storage capacity and balance on hand for
        water in organizational bulk water storage devices; i.e., stave
        tanks, bladders, onion tanks, etc. (more than 500 gal
        capacity). Round to the nearest thousand gallons.

        Note 6. Show location's average water consumption factor
        (gallons per person per day). Factor should be based upon
        comparison of estimate of daily water consumed with record of
        daily population supported.

        Note 7. Calculate days of supply (DOS) by comparing the
        quantity of water on hand with the estimated consumption of the
        projected population to be supported. If the projected
        population changes significantly each day, the on-hand balance
        should be compared successively to each day's estimated
        consumption figure rather than to a single, unchanging daily
        consumption estimate.

        Note 8. Considering anticipated changes in sourcing,
        production, storage, distribution, or consumption and projected
        changes in population, maintenance, and operations, state if
        water status is considered satisfactory or unsatisfactory for
        the next 5-day period. Impact should be addressed in
        paragraphs 2a(3) and 2a(4), as required.

        Note 9. For each operating location, list nomenclature and
        quantity of organizational water purification, storage,
        distribution, and chiller equipment that is available. (At
        joint service operating locations, units should list only their
        own equipment and not that of other services, who will be
