           1. Cpt Thomas Gammarano was approached at the pharmacy
           window by a soldier who was redeploying back to the states
           and wanted to know if we would like to have their
           medications.

           2. Cpt Thomas Gammarano took two boxes of medications from
           the soldier to use as our stock.

           3.  Contained in the two boxes was a package of Diazepam 5mg
           I.S. tablets. Lot#1533-028. Expiration date:6/92

           4.  The package contained 100 individually sealed tablets.

           5.  The medication was logged into the controlled drug stock
           record and added to the pharmacies stock of Diazopem: k4g
