             SUBJECT:  Rule Out Samonella Food Poisoning Outbreak Coupled
                       with Viral Gastroenteritis



                      Refrigeration units had appropriate temperatures,
             however, they were dirty with dried blood noted in meat storage
             and food Stuff was not palatized.

                  4 . The sump pits are located too close to the food
             preparation areas and is over-filled, and stagnant water is on
             the premises.   Logistical coordination was discussed and gravel
             was procured from 4/27 FA to cover stagnant water with earth.

                  5.  The perimeter smelled of urine.     Recommended monitoring
             of personnel.

                  6.  The chilling unit for thawing of meats was not avail-Able
             for inspection as it was "giving them trouble" on or about the
             10 of April 1991.   Furthermore, on the 11 of April 1991 they were
             cut two (2) hours of preparation time for the chic:t.-.en dinner-.

             S.  All findings were correlated and presented to Dr. John
             Falcon, CPT, NC 2iOth FA Brigade Surgeon with R/O Samonelia Food
             Poisoning.  On I--, April 1991, we both went to re-inspect Hanarkd
             Catering, with resolution of f. 1, 2.     A Preventive Medicine Team
             headed by SSG Ingram inspected 4/"@7 FA (MLRS) and concurred with
             my findings and outstanding set-tip.

             6.  Currently patients are presenting with the same symptoms as
             the patients in paragraph one (1) with resolution in 99% Of th-
             patients.  Those without resolution have been escorted tc) the
             hospital for stool specimen collection as at this location
             logistically it is next to impossible, secondary to the majority
             of its sets, lits, and outfits, equipment end Supplies        e
             loaded for re-deployment.
