                                         COMMAND INTEREST
                   OCCURENCES NOT INVOLVING LOST TIME THAT MUST BE REPORTED

              All accidents, with or without lost time, regardless of     dollar   damage
        amount,  dealing with the following, will be reported to      the Safety Office:

              a.  Heat or Cold Weather injuries. (Tally) Must be determined If heat
        injured  personnel have had previous heat injuries.

              b.  Split Rim -Aire Mount Incidents.

              C.  Insect/Snake Bites.

              d.  Turret and/or Hatch Cover Injuries     (Was hatch cover modified, was
        injury due to missing pin)

              C.  Injuries   due   to  Improper  Construction   of   Fighting Positions.

              f.  Injuries   due   to  Sleeping in Unauthorized Area or Improper Sleep
        Procedures while on tank crew.

              g.  Blackout Reduced Visibility Motor Vehicle Accidents/Incidents.

              h.  Vehicle Fires (state whether fire extinguishers were available and
        used, and if they were Internal or external)

              I.  All fire3 regardless of velocity or involvment.

        2.   All accidents, with or without    lost time,   dealing   with the following,
        will  be reported to the Safety Office on a DA Form 285:

              a.  Noise-Induced hearing loss.

              b.  AsbestO3i3.

              C.  Effects of radiation exposure.

              d.  Toxic chemical exposure.

              e.  DermatO3i3.

              f.  SiliCOSL3.

              g.  Sight loss or impairment due to exposure to laser, are welding, or
        other intensified light source.

              h.  Explosives mishap to Include pyrotechnic simulators. (Include type
        of pyro and model number.)
