   3D 10 (DAILY RECEIVING AND DISPOSITION REFORTI (R&D)

   1. GENERAL. The RED 16 (R&D) report will be prepared by all clearing       station and
   higher echelon medical treatment facilities. The R&D report is designed to
   facilitate transmittal of unit casualty information significant to tactical
   operations.

   2. REPORTING UNITS.


        a. Medical Clearing Companies submit reports to the following addresses      RLT
   1400 hours daily:

            (1) DISCOM MEDICAL OPERATIONS CENTER

            (2) BDE LEVEL (or.ieparate battalion) S-1

        b. The DISCOM Medical Operations Center will consolidate clearing station
   reports and report to the Division Surgeon in D-  my-   ULT 1600 hours daily.
   3. TRANSMISSION. Ttio RED 16 (R&D) report will be transmitted by the Moat
   expeditioua means available to include secure telephone, teletype, radio-teletype      or
   messenger. When transmitted by electrical meang,'the report will be subject to the
   game security considerations afforded other tac@ical massages.

   4. PERIOD. Report will be submitted daily as of 1200 to arrive to the Division
   lurgeon D-MAIN ULT 1600 daily.

   5.   FORMAT.


        CODE                 ITEM

        ALPHA                Medical unit reporting.
        BRAVO                DTO
        CHARLIE              SSM of each casualty.
        DELTA                Bank/Grade of each casualty.
        ECHO                 Surname and initials of each casualty.
        FOXTROT              Unit of each casualty.
        GOLF                 Nationality of casualty's unit.
        HOTEL                Diagnosis (to include VSI) SI and/or logs of hand,
                             foot, limb or eye.
        INDIA                Categorization (*battle casualty: BC, non-babtlo
                             accident/injury: NBA/NBI, or sick/digeaso: S/D).
        JULIET               Dates of admission, transfer, or digcharge/RTD.
  Unit which casualty transferred or RTD.
        LIMA                 If deceased, indicate died and DTO.

   NOTE: Omit lines not applicable.
