1. Re your fax pertaining to the Guest, Mitchell, and Payne inju-
ries - there were several things that contributed to the confusion
concerning notification to OOK on these three cases.

a. Initial policy was that DA Casualty would telephonically
notify PNOK of soldiers who were reported as VSI/SI/SPEC CAT/WIA.
DA changed their policy, indicating they would telephonically no-
tify PNOK of WIA only if classified as VSIISIISPEC CAT.' This pol-
icy flip-flopped several times during the period 16-25 Feb 91.
Unfortunately, formal announcements of the policy change was not
made and we were informed only in the process of our many calls to
DA to confirm PNOK notification as we were working the casualty
reports.
b. Initial report on Guest listed him as DRIVE (SPEC CAT).
We received the report on Guest at approximately 0819 hrs, 18 Feb
DA called and confirmed their notification to PNOK. We then
advised Unit Contact Team.
C. Initial report on Mitchell listed him as ABBOTT (treated
and returned 'to duty). We received casualty report on Mitchell at
approximately 1053 hrs, 18 Feb 91 - DA called and confirmed their
notification to PNOK. Unit Contact Team advised.@ UCT called lat-
er that evening and said Mrs. Payne had not been notified by DA.
We called DA back and they said of the 3 (Mitchell, Payne, &
Guest) - only the,NOK on Guest had actually been notified based on
change in policy. :.UCT advised so they could notify PNOK. Because
PNOK did not have telephone, notification was not possible by UCT
sed that DA had in fact
notified her on 19 Feb 91.@@
d. The initial casualty report received on Payne reflected
his status as FRIAR (Not Si hospital care required). Casualty
sup 1 dtd 22 Feb 91 changed status to GAVEL (SI).. We received
casualty report on Payne at approximately 1100 hrs, 18 Feb 91 - DA
called and confirmed their notification to POOK - Unit Contact
Team advised. UCT called later that evening and said Mrs. Payne
had not been notified by DA. We called DA back and they said of
the 3 (Mitchell, Payne, & Guest) - only the NOK on Guest had ac-
tually been notified based on change,in policy. UCT advised so
they could notify PNOK. We questioned DA about this case because
even though Payne was listed as FRIAR -- his injuries sounded same
as Guest and they had notified PNOK on Guest. ;DA then called us
on 19 Feb 91 and said they had reconsidered 'and.would make tele-
phone notification. DA was advised PNOK 'already,notified thru
UCT. DA did contact PNOK after-the-fa.ct. DA also provided fol-
low-up telephonic notification to PNOK on 22 Feb based on supple-
mental report.
