 tank caught fire; successful ejection on the ground. 
 
7. F-16 Night interdiction training, following turn on heading 35 miles from the 
target called "terminate", impacted the ground, probable spatial disorientation; 1 
fatality. 
 
8. B-52G Night return to base following 30-hour duty day and bombing run, 
multiple human factors, fuel valve failure and 2-engine flameout misdiagnosed, 
procedural errors resulted in double generator failure and multiple engine 
flameout, delayed decision to eject; 3 fatalities, 3 successful ejections. 
 
9. F-16 Night ASLAR approach to base after third sortie of the day, impacted the 
ground, probable spatial disorientation; 1 fatality. 
 
10. F-16C Engine failure/fire; successful ejection 
 
11. F-4G Fuel starvation secondary to battle damage on return to base; 2 
successful ejections. 
 
12. F-16C Ordinance malfunction, structural damage to wing; successful ejection 
over water, recovered by USN SAR. 
 
13. EF-111 Night mission, inexperienced pilot, defensive reaction to friendly radar 
lock-on, impacted the ground; 2 fatalities. 
 
14. OA-10 Hydraulic failure secondary to battle damage, attempted manual 
reversion landing against Dash 1 recommendation (successfully accomplished by 
another pilot the day prior), hard landing, gear collapsed, aircraft flipped; 1 fatality.
